• If you would like to use your health insurance to cover your service, please contact me in advance to verify if your insurance covers my services. 

  • When you contact me please provide with following information: 

    • Name of your Insurance Provider

    • Your Insurance ID

    • Insurance Company’s phone number 

    • Your full name 

    • Your birthdate 

    • Your home address 

    • If you are a spouse or dependent of the primary insurance holder, please give us the primary insurance holder’s name and birthdate. 

 

• I will do my best to verify your coverage for nutrition therapy, however, I strongly urge you to check with your insurance provider to make sure your plan covers for nutrition services before making an appointment with me. 

• You can find the customer service number on the back of your insurance card. 

 

Here are key questions to ask your insurance provider: 

1. Does my insurance plan cover In-Network nutrition therapy? 

2. If I have coverage, do I need a prescription from the primary care physician? 

3. Do I have any limitations on the number of visits or length of time for each visit?

4. Has the deductible been met? If not, how much would they cover for the service?

5. Is there a copay or coinsurance for the visit? If yes, how much? 

6. What is the maximum amount payable per policy term?

7. Is there a maximum reimbursement amount per visit?
